Position Summary
We have an exciting opportunity to join our team as a Program Supervisor for Early Childhood Initiatives. The Program Supervisor will provide supervision to community health workers and counselors who support families with children under five years old. The Program Supervisor will work closely with Maternal Infant Health programs and the FHC data team to support program integration and continuous quality improvement.

Job Responsibilities

Provide reflective supervision for community health workers and counselors.

Provide ongoing training and support for community health workers.

Review documentation of community health workers to ensure compliance with guidelines.

Identify resources and develop partnerships to assist patients in addressing social needs and barriers to care.

Review program data for continuous program improvement.

Ensure integration of community health workers with members of the care team, FSS department, and maternal infant health programs.

Collaborate with the Quality Team and supervisor to develop targets and strategies to meet targets.

Support community health workers and counselors to ensure that they screen clients for social determinants of health and address identified needs.

Monitor billing and documentation related to the department’s participation in the 1115 waiver.

Complete the monthly and quarterly reports for the MIH grant and other maternal infant health grants.

Minimum Qualifications

Bachelor’s degree and at least two years of experience as a community health worker or case manager/counselor.

Preferred Qualifications

Supervisory experience preferred, but not required.

Ability to effectively communicate with all levels of the organization.

Salary
The salary range for this role is $66,300.00 – $70,000.00 annually. Actual salaries depend on experience, specialty, education, and hospital need. The listed range does not include bonuses, differential pay, or other compensation.
