Job Overview
Job Description: The role involves performing both social work and discharge planning functions. The position is available immediately and offers a floating schedule, with occasional weekend requirements (typically one full weekend per four weeks, subject to departmental need).

Responsibilities

Provide comprehensive social work services to patients, including assessment, planning, and intervention.

Coordinate discharge planning activities to ensure smooth transitions of care.

Adhere to project timelines and staffing schedules, including on-call periods as needed.

Qualifications

Must hold a California state license as a Licensed Clinical Social Worker (LCSW) or be eligible for licensure upon hire.

Minimum of 2-year acute medical social work experience is preferred.

Ability to pass a first-time traveler screening if required.

Prior experience with acute care facilities is an asset.

Schedule

Full-time position with floating days and occasional weekend coverage.
