About The Role The RN Field Coordinator (RN FC) supports on‑ground continuity of care data gathering and the facilitation of effective onboarding of new health plan members and new facilities. The RN FC works with local clinical staff and leadership to educate and train new nursing facilities on the clinical model, accesses facility and LH EMR data to ensure the LH clinical team understand member transition needs, and communicates any needs for network or provider support to the National Care Coordination Team. The RN FC helps gather information for NC NPs during onboarding and facilitates initial visits for the NPs. The RN FC will travel to nursing homes upon request and assignment by the Market Director of Clinical Services (MDCS).
Key Tasks and Responsibilities
Works directly with the clinical and sales team to identify member external appointments, facilitates access to those providers in a seamless and timely manner, and communicates back to the member’s NP and NH contact.
Uses facility EMRs to pre‑screen new enrollees to determine any specific continuity of care needs for external appointments/specialists, and facilitates their access.
Performs care coordination activities – member transitions of care communication to and from facility, assists with Prior Auth requests, assists with members needing out‑of‑Nursing Facility care and services, knows how to access the drug formulary, provider directory and communicates patient/family and doctors as delegated by team‑Nurse Practitioners.
Participates in Longevity Health Plan NC standing clinical meetings on a daily/weekly/monthly basis.
Assists with gathering data for hospitalization review sessions.
Assists with onboarding new nursing homes, educating team members on LHP model.
Assists with the initiation of telehealth service in newly onboarded NHs, assisting with education and following up as needed.
Proficient user of the LHP EMR.
Conducts nursing home orientation sessions for all shifts on LHP MOC and introduction to the model on all new homes and refreshers on existing homes in the Market.
Assists with LHP EMR data input upon request of MDCS when a team member has difficulty or is behind.
Performs other duties as assigned, or as required by the team and based on caseload, workload, etc.
Supervisory Responsibilities
There are no supervisory responsibilities for this position.
Credentials & Coverage
Licensed registered professional nurse in good standing.
Valid state driver’s license with a good driving record and proof of automobile insurance required.
Auto insurance coverage as required by your state of residence; new employees must provide proof of coverage prior to hire and on an ongoing basis.
Education and Training
Associate degree in nursing required; bachelor’s degree in nursing or other healthcare‑related discipline acceptable if licensed RN.
Knowledge and Experience
Background in nursing homes or health plan case management preferred.
Minimum 2 years’ experience as a Registered Nurse.
Excellent verbal and written communication skills.
Excellent computer skills, EMR/clinical platform/MS Office proficiency.
Knowledge of challenges and opportunities in long‑term nursing home population.
Able to travel.
Required to have managed care coordination background/experience.
