About The Role The RN Field Coordinator (RN FC) supports on‑ground continuity of care data gathering and facilitates the onboarding of new health‑plan members and facilities. Working with local clinical staff and leadership, the RN FC trains new nursing facilities on our clinical model, reviews EMR data, communicates member transition needs, and coordinates with the National Care Coordination Team. The RN FC may conduct visits to nursing homes, assist with telehealth initiation, and help gather data for onboarding new members.

Key Responsibilities

Work directly with clinical and sales teams to identify member external appointments and facilitate timely access to providers.

Use facility EMRs to prescreen new enrollees and determine specific continuity of care needs.

Perform care‑coordination activities—including member transition communication, prior‑auth requests, and assistance with nursing‑facility‑related services.

Participate in Longevity Health NC standing clinical meetings on a daily/weekly/monthly basis.

Assist with gathering data for hospitalization review sessions.

Onboard new nursing homes, educating team members on the LHP model.

Assist with the initiation of telehealth services in newly onboarded NHs, including education and follow‑up.

Proficiently use the LHP EMR and provide data input support when requested by the Market Director of Clinical Services (MDCS).

Conduct nursing‑home orientation sessions for all shifts on LHP MOC and introduce the model on new homes while providing refreshers on existing homes in the market.

Perform other duties as assigned based on caseload and workload.

Supervisory Responsibilities

There are no supervisory responsibilities for this position.

Credentials & Coverage

Licensed registered professional nurse in good standing.

Valid state driver’s license with a good driving record and proof of automobile insurance.

Auto insurance coverage required by state of residence.

Education & Training

Associate degree in nursing required; bachelor’s degree in nursing or another healthcare‑related discipline preferred, provided the candidate is a licensed professional registered nurse.

Knowledge & Experience

Background in nursing homes or health‑plan case management preferred.

Minimum two years experience as a Registered Nurse.

Excellent verbal and written communication skills.

Strong computer skills, including EMR/clinical platform and MS Office products.

Knowledge of the challenges and opportunities serving the long‑term nursing home population.

Ability to travel as required.

Prior managed‑care coordination experience required.
