The RN Field Coordinator (RN FC) supports on‑ground continuity of care data gathering and facilitates the effective onboarding of new health plan members and new buildings. The RN FC works with local clinical staff and leadership to educate and train new nursing facilities on the clinical model, accesses facility and LH EMR data to ensure LH clinical team members understand member transition needs, and communicates to the National Care Coordination Team any needs for network or other provider support. The RN FC also helps gather additional information for the NC NPs who are onboarding new members and facilitates initial visits for the NPs. The RN FC travels to different nursing homes upon request and assignment by the Market Director of Clinical Services (MDCS). Key Tasks and Responsibilities
Works directly with the clinical and sales team to identify member external appointments, then facilitates access to those providers in a seamless and timely manner and communicates back to the member’s NP and NH contact. Uses facility EMRs to prescreen new enrollees to determine any specific continuity of care needs for external appointments or specialists and facilitates their access. Performs care‑coordination activities – member transitions of care communication to and from facility, assists with prior‑auth requests, assists with members needing out of nursing‑facility care and services, knows how to access the drug formulary and provider directory, and communicates with member patients, families and doctors as delegated by team nurses and practitioners. Participates in Longevity Health Plan NC standing clinical meetings on a daily, weekly or monthly basis. Assists with gathering data for hospitalization review sessions. Assists with onboarding new nursing homes, educating team members on the LHP model. Assists with the initiation of telehealth service in newly onboarded nursing homes, providing education and following up as needed. Is a proficient user of the LHP EMR. Conducts nursing‑home orientation sessions for all shifts on LHP MOC and introduces the model on all new homes with refreshers for existing homes in the Market. Assists with LHP EMR data input upon request of the MDCS when a team member encounters difficulty or is behind. Performs other duties as assigned or as required by the team based on caseload and workload. Supervisory Responsibilities
There are no supervisory responsibilities for this position. Credentials & Coverage
Licensed registered professional nurse in good standing. Valid state driver’s license with a good driving record and proof of automobile insurance required. Auto insurance coverage as required by your state of residence; new employees must provide proof of coverage prior to hire and on an ongoing basis. Education and Training
Associate degree in nursing required; bachelor’s degree in nursing or another health‑care related discipline is preferred, but a licensed professional registered nurse is required. Knowledge and Experience
Nursing‑home background or health‑plan case‑management experience preferred. Minimum two years of experience as a registered nurse. Excellent verbal and written communication skills. Excellent computer skills, including EMR/clinical platform and Microsoft Office products. Knowledge of the challenges and opportunities to serve the long‑term nursing‑home population. Ability to travel. Required to have managed‑care coordination background or experience. Company Overview
