As a Home Health Aide on our Hospice team, you will care for patients where they need you most, whether at home, a nursing facility, assisted living center or other location. In this role, you can help enhance their outlook on their experience, even when their futures seem uncertain. Every day, you can make a lasting difference in both the lives of your patients and their loved ones./ppA caregiver in this position works nights, 9:00pm1:00am or 11:00pm-7:30am, no weekends required./ppA caregiver who excels in this role will:/pulliPerform basic patient care activities./liliAssist with procedures ordered by physician, with supervision from an RN, PT, OT or ST as appropriate./liliAssist patients with tending to personal care, activities of daily living and transfers/transport./liliReport and document abnormal assessment findings or changes in physical, mental and emotional conditions to nursing staff./liliAssist with keeping the patients environment clean and orderly./liliMaintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions./liliEscalate change in patient condition to patients physician as appropriate./li/ulpMinimum qualifications for the ideal future caregiver include:/pulliHigh School Diploma or GED/liliState Tested Nurse Aide (STNA) Certification with the State of Ohio Nurses Aide Registry/liliOne year of clinical experience as an Aide in an acute, rehab or skilled nursing setting/liliValid Drivers License (VDL) in Ohio and proof of $100,000/$300,000 automobile insurance coverage/liliBasic Life Support (BLS) Certification through the American Heart Association (AHA) within orientation period/li/ulpPreferred qualifications for the ideal future caregiver include:/pulliHospice and Palliative Care Nurse Assistant (CHPNA) Certification/liliHome Care experience/li/ulpPhysical Requirements:/pulliMust be ambulatory, able to climb stairs, pull, push, and facilitate movement of the patient./liliFor field staff only: Able to drive to patients homes and/or facilities./liliMedium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects./liliPhysical Demand requirements are in excess of those for Light Work./li/ulpPersonal Protective Equipment:/pulliFollows standard precautions using personal protective equipment as required./li/ulpPay Range/ppMinimum hourly: $18.25/ppMaximum hourly: $24.95/ppThe pay range displayed on this job posting reflects the anticipated range for new hires. A successful candidates actual compensation will be determined after taking factors into consideration such as the candidates work history, experience, skill set and education.
