
Program Director Care Coordination

Comunilife is looking for an experienced Human Services Director for its new Care Coordination Program in the Bronx and Brooklyn. The Director shall ensure all elements of Health Home care coordination focus on achieving and setting New York State Department of Health measurable goals through quality‑driven, cost‑effective, culturally appropriate, person‑ and family‑centered care coordination. The Director shall ensure the efficient use of health information technology so staff link clients to needed services, facilitate communication among clinical team members, individuals and family caregivers, and provide feedback on feasible and appropriate practices. The Director shall ensure the timely and comprehensive submission of billing information to the health home and coordinate with fiscal and others on billing and payment issues. The Director will further establish a continuous quality improvement program, and collect and report on data that permit an evaluation of increased coordination of care and chronic disease management on individual‑level clinical outcomes, experience of care outcomes, and quality of care outcomes at the population level.

Duties and Responsibilities
- Oversight of daily program operations.
- Complete and submit required reports to Lead Health Homes as noted in the Business Associate Agreements.
- Review staff electronic daily activity logs to ensure effective time management, minimum required patient contacts are met and to identify potential concerns.
- Ensure compliance with the NYSDOH Data Exchange Agreement on confidentiality.
- Review and resolve all personnel related activities such as approving personal time, completing bi‑weekly payroll, disciplinary and termination activities and ensuring compliance with Comunilife Policy & Procedures.
- Review and resolve all fiscal programmatic needs.
- Ensure that all submissions to the health home for billing are timely and complete.
- Work with fiscal to resolve any billing and/or payment issues.
- Evaluate staff cohesion and resolve issues and concerns.
- Ensure development and implementation of QI activities.
- Ensure quality clinical services are performed within the spectrum of NYSDOH guidelines.
- Attend Health Home Network Provider meetings as needed.
- Provide support to clinical staff as needed.
- Participate in the development and update of company’s Health Home Policy & Procedures.
- Provide oversight in patient related interventions to ensure service provisions remain in a continuum.
- Ensure all program staff upholds the agency's Code of Conduct/Code of Ethics and complies with all Federal, state and local law/regulation, including 42 CFR confidentiality and HIPAA privacy and security regulations.
- Perform other duties as assigned by supervisor.
Education and Experience Requirements
- Master’s Degree in Human Services or related field, required.
- LMSW, preferred.
- Must have at least 1–3 years’ experience working in a Health Home program at a leadership capacity.
- Must have at least 3 years’ experience in supervising staff that provides direct services to individuals with mental illness and substance use disorders.
- Working knowledge of HARP, required.
- General understanding of chronic conditions i.e. HIV, substance abuse, and mental illness.
- Must be proficient in Microsoft Office Suite.
- Bilingual-Spanish, preferred.
