Arcola Creek Health & Rehabilitation, a BRAND-NEW skilled nursing facility located in Madison, Ohio, is seeking a Full-Time, Salaried Social Services Director to join our dedicated management team. We are seeking an empathetic individual to provide support and assistance to residents and families facing various challenges. The Social Services Director is responsible for managing and/or assisting with various resident services including MDS, PASSR requirements, Ancillary Services, Level of Care (LOC) and Medicaid eligibility requirements as well as discharge coordination, care conferences, and assisting with quality assurance for resident services.
Responsibilities
Manage and/or assist with resident services including MDS, PASSR requirements, Ancillary Services, Level of Care (LOC) and Medicaid eligibility requirements.
Coordinate discharges and participate in care conferences.
Assist with quality assurance for resident services.
