Overview
Lead a dynamic 16‑bed Transitional Care Unit where your leadership directly shapes each patient’s path to recovery and renewed independence. Inspire and guide a skilled, compassionate team dedicated to helping patients regain strength, confidence, and the abilities needed for everyday living.
Responsibilities
- Direct and coordinate all HB/SNC Unit nursing activities per the PA Nurse Practice Act.
- Monitor care quality and documentation accuracy.
- Develop and maintain unit Standards of Care, practice guidelines, and nursing policies.
- Manage staff performance, competencies, hiring, retention, and education.
- Oversee budgeting, staffing plans, and resource allocation.
- Maintain effective communication and lead departmental meetings.
- Collaborate with leadership on resident care programs and policy development.
- Lead Quality Assurance/Continuous Improvement efforts.
- Ensure coordinated discharge planning and patient/family teaching.
- Investigate incidents, enforce corrective actions, and ensure safety compliance.
- Maintain clinical expertise and serve as a professional resource.
- Develop job descriptions and provide orientation for nursing personnel
Qualifications
- Completion of an accredited School of Nursing required.
- BSN preferred.
- 3–5 years of gerontology/medical-surgical nursing experience.
- 2–3 years of direct supervisory or management experience.
- Knowledge of general nursing theory and practice, as well as administrative methods and procedures.
- Working knowledge of public relations and personnel management
