Intake Specialist , you will be the first point of contact, guiding clients through the eligibility, assessment, and intake process and connecting them to life-changing resources.
Key Responsibilities
Conduct client screenings, eligibility assessments, and intakes for housing and supportive service programs
Complete intake documentation, enrollment, and discharge processes accurately and efficiently
Enter and manage data in HMIS (Homeless Management Information System) and internal databases (MCR)
Track client services, program participation, and outcomes for reporting purposes
Facilitate client orientations and explain program guidelines and expectations
Collaborate with case managers, outreach teams, and housing specialists to coordinate services
Assist in preparing monthly, quarterly, and annual program reports
Maintain confidential client records in compliance with HIPAA and program standards
Support aftercare follow-up and ongoing client engagement
Assist with program operations, supplies, and administrative support
Program-Specific (SSVF / GPD)
Support Supportive Services for Veteran Families (SSVF) or Grant & Per Diem (GPD) housing programs
Assist with temporary financial assistance tracking, payment processing, and fund documentation (SSVF)
Qualifications
Bachelor’s degree in Social Work, Human Services, Psychology, or related field (preferred)
1+ year of experience in social services, case management support, or intake coordination
Experience working with homeless populations, veterans, or housing programs (preferred)
Knowledge of HMIS, data entry, and program reporting
Strong organizational, administrative, and documentation skills
Excellent communication and client engagement skills
Proficiency in Microsoft Office and data systems
High attention to detail and ability to manage multiple priorities
Valid driver’s license and ability to meet insurance requirements
