Job Title: Hospice Referral Intake Specialist (Part-Time or Full-Time)

$20.00 – $25.00 per hour (based on experience)
About the Role We are seeking a compassionate and detail-oriented Hospice Referral Intake Specialist to serve as the first point of contact for patients, families, and healthcare partners. This role is essential in ensuring timely, coordinated, and supportive access to hospice care.
This position offers flexible scheduling allowing team members to build a schedule that fits their availability while ensuring consistent 7-day coverage.
Available Shifts Candidates may select from the following shift options to build a schedule of:
Part-Time:
Up to 24 hours/week
Full-Time:
Up to 32–40 hours/week
Shift Options Include:
9:30 AM – 4:00 PM (6-hour shift)
8:30 AM – 12:30 PM (4-hour shift)
12:30 PM – 4:30 PM (4-hour shift)
Shifts are available 7 days per week, including weekends, to cover patient referral needs. Schedules are built collaboratively and may include a mix of weekday and weekend coverage.
Key Responsibilities
Referral Processing:
Receive and manage hospice referrals, ensuring all required documentation is complete and accurate
Patient & Family Communication:
Provide clear, compassionate guidance on hospice services, eligibility, and next steps
Care Coordination:
Collaborate with clinical teams, social workers, and external providers to ensure smooth admissions
Insurance Verification:
Confirm coverage, benefits, and eligibility for hospice services
Compliance:
Maintain adherence to Medicare, Medicaid, and state hospice regulations
Documentation:
Accurately enter and maintain patient information in electronic medical systems
Qualifications
High school diploma required; Associate's or Bachelor's degree in healthcare or related field preferred
1–2 years of experience in healthcare intake, hospice, home health, or similar setting (hospice experience preferred)
Healthcare experience, especially in patient-facing roles like CNA or Medical Assistant, is a strong plus
Strong communication and interpersonal skills
Proficiency with electronic medical records (EMR) systems
Ability to manage multiple priorities and coordinate across teams
