Licensed Social Work Director

to join our team. This is an exciting opportunity to join our mission-driven organization to provide excellent care to our patients. This role will manage our long term care and rehab social work team.

Social Work Director (LCSW or higher) The Social Work Director is responsible for the overall leadership and organization of the Social Work department. This position will provide administrative and clinical supervision to social workers and discharge planners to ensure quality of service to the long term care and rehabilitation units for Stella Maris. Services include admission, psychosocial assessment, plan of care, linkage with community resources, counseling, crisis intervention, and discharge planning.

This position will manage the team of Social Workers. Duties will include payroll/timekeeping for SW staff, performance evaluations, point of contact for questions and escalations, hiring, and general management of the department. This position will provide direct social work services to nursing home and rehabilitation patients and families as needed. The Director also ensures the departmental documentation is in compliance with State and federal regulations for the department.

Responsibilities

Responsible for planning, developing, organizing, implementing, evaluating and directing the operations of social service programs for the facility.

Promotes the mission, vision, and values of Stella Maris.

Develop, plan and implement program goals, objectives, personnel, resources and equipment.

Develop and implement policies and procedures for identifying the medically-related social and emotional needs of the patients/residents.

Assists in providing solutions to social and practical environmental problems, including seeking financial assistance, discharge planning (including collaboration with community agencies) and providing referrals to other community agencies when specialized assistance is required.

Provides supervision to staff that meets regulatory compliance.

Provide clinical consultation, oversight of case planning and care coordination.

Responsible for a caseload.

Direct crisis intervention efforts and intervene as required.

Provide staff management to include hiring, development, training, performance management and communication to ensure effective and efficient department operation.

Ensure procedures and workflow are consistent with organizational objectives, treatment protocols, regulations and standards of care.

Ensures regulatory compliance with MDS, BIMS, PHQ9, Care planning.

Requirements

Licensed SW in the State of Maryland required (LCSW, LMSW, LCSW-C).

Degree in SW required.

Minimum of 1 year Medical Facility experience preferred (Nursing Home or Hospital).

Management Experience Preferred.

Experience in Adult or Geriatric population preferred.
