This executive-level leadership role is responsible for aligning clinical excellence, regulatory compliance, and financial stewardship while driving measurable outcomes across interdisciplinary teams. This opportunity is well-suited for a seasoned RN or Social Work leader with a strong operational background in acute care case management who is ready to influence system-wide performance and patient flow. Responsibilities include providing strategic and operational leadership for all hospital case management functions, directing daily departmental operations, ensuring effective care coordination and discharge planning processes, overseeing documentation standards, collaborating with executive leadership, analyzing performance data, maintaining compliance with regulatory requirements, and fostering strong relationships with various partners. Required qualifications include a bachelor's degree, active licensure as a Registered Nurse (RN), Licensed Clinical Social Worker (LCSW), or Licensed Master Social Worker (LMSW), minimum 3 years of acute care hospital case management experience, and minimum 2 years of case management leadership experience in an acute care setting. Preferred qualifications include a master's degree in Nursing, Health Administration, or Business Administration, experience leading case management programs in high-volume or complex hospital environments, and strong financial acumen and performance improvement expertise.
