Provide patient care and support in a skilled nursing environment. Collaborate with healthcare teams to develop and implement patient care plans. Administer medications and treatments as prescribed. Monitor patient progress and report any changes in condition. Educate patients and families on health management and rehabilitation. Maintain accurate patient records and documentation.
