Key Responsibilities
Conduct structured intake calls with new members to build trust and set expectations Proactively manage patient panels to maximize program completion and appointment attendance Respond to same-day cancellations to fill open slots and minimize idle clinical time
Required Qualifications
3+ years in member engagement, patient outreach, or care coordination in a virtual care or digital health setting Proven track record of driving measurable outcomes in appointment adherence and program completion Experience with multi-channel outreach including phone calls, SMS, and direct mail Proficiency in CRM and EHR platforms, with a strong preference for experience in eClinicalWorks Comfortable using Google Sheets for tracking and reporting data
