Scheduled Weekly Hours: 40
Job Summary Responsible for overseeing and coordinating community health programs and activities, providing leadership, training, and guidance to Community Health staff. Programs may include exercise, mental health, medication management, nutrition, health care system navigation, substance use, and other health behaviors. The role collaborates with healthcare providers to ensure patients receive appropriate care and support services, improving health outcomes. The incumbent may also allocate work and manage staff in the absence of the Manager.
Essential Functions
Collaborate with healthcare providers, community organizations, and other stakeholders to develop and implement effective strategies that improve health outcomes.
Ensure compliance with regulatory requirements and program standards.
Collect and analyze data to measure program effectiveness and identify opportunities for improvement.
Assist families in applying for social services, including Medicaid and WIC.
Provide daily contact and resource navigation peer support to community health workers.
Direct patients/families to additional resources as needed.
Maintain participant charts and EHR updates, providing progress notes according to organizational standards.
Counsel clients in individual or group sessions to assist them in dealing with substance abuse, mental or physical illness, poverty, unemployment, or physical abuse.
Assist family members in understanding, dealing with, or supporting the client or patient.
Qualifications Education requirements: Bachelor's Degree in Public Health, Social Work, or a related field.
Experience: 3-5 years in community outreach, health education, or a related field (experience recognised in lieu of a degree is acceptable).
Strong communication and interpersonal skills for interacting with various populations.
Ability to work independently and as part of a team.
Strong organizational and project management skills.
Basic computer skills, including Microsoft Office and database management.
Bilingual skills preferred.
