
Lead Community Health Worker

Summary
Responsible for overseeing and coordinating community health programs and activities, as well as providing leadership, training and guidance to Community Health staff. Areas could include exercise, mental health, medication management, nutrition, health care system navigation, substance use and other health behaviors. They work closely with healthcare providers to ensure that patients receive appropriate care and support services to improve health outcomes. Will allocate work and manage staff in absence of Manager. Does this position require Patient Care? Yes.
Essential Functions
Collaborate with healthcare providers, community organizations, and other stakeholders to develop and implement effective strategies to improve health outcomes
Ensure compliance with regulatory requirements and program standards
Collect and analyze data to measure program effectiveness and identify opportunities for improvement
Assist families to apply for social services, including Medicaid or Women, Infants, and Children (WIC)
Available for daily contact with CHWs to provide resource navigation peer support
Direct Patient/family to additional resources as needed
Updates and inputs progress notes, participant charts and EHR according to organizational standards
Counsel clients in individual or group sessions to assist them in dealing with substance abuse, mental or physical illness, poverty, unemployment, or physical abuse
Counsel or aid family members to assist them in understanding, dealing with, or supporting the client or patient
Education Bachelor's Degree Public Health required or Bachelor's Degree Social Work required or Bachelor's Degree Related Field of Study required.

Experience Experience in community outreach, health education, or related field 3-5 years required.

Strong organizational and project management skills
Basic computer skills, including Microsoft Office and database management
