This is a high-impact leadership opportunity for an experienced Case Management leader to oversee operations across a large, high-acuity acute care hospital. You will take ownership of care coordination, discharge planning, and resource utilization across a complex, fast-paced environment, driving outcomes through strong interdisciplinary collaboration and data-driven performance improvement.
Overview Reporting to the VP of Case Management, this Director will lead all aspects of Case Management, Social Work, and Chaplain services across a 790-bed, Level II Trauma facility. This is a visible leadership role responsible for aligning clinical and operational strategy, optimizing patient flow, and ensuring high-quality, compliant care coordination across the continuum.
What You Will Do
Provide strategic and operational leadership for Case Management, Social Work, and Chaplaincy services
Oversee day-to-day operations including staffing, workflows, and performance management across ~110 FTEs
Lead and develop a leadership team (4 Managers, Social Work Supervisor, Administrative support)
Drive care coordination and discharge planning processes to ensure timely, compliant, and patient-centered outcomes
Partner closely with executive leadership, physicians, and post-acute providers to improve throughput and resource utilization
Analyze operational and clinical data to identify gaps and implement sustainable performance improvement initiatives
Ensure compliance with regulatory standards, policies, and best practices
Collaborate with CFO and system leadership on departmental goals, financial performance, and strategic initiatives
Support a high-performing culture focused on accountability, communication, and continuous improvement
What You Bring
Bachelor's degree required; Master's preferred (Nursing, Healthcare Administration, or Business)
RN, LCSW, or LMSW required
3+ years of acute care hospital Case Management experience
2+ years of leadership experience in Case Management
Proven ability to lead large, complex teams and drive measurable outcomes
Strong analytical skills with the ability to translate data into actionable plans
Executive presence with excellent communication and relationship-building skills
Experience collaborating with physicians and post-acute networks
