Patient Engagement Specialist I – Overview Under general supervision, provides patient engagement services to recruit patients into the program’s system of care. The Patient Engagement Specialist I participates in, and supports, the planning, development, implementation, and evaluation of services in accordance with contractual and departmental requirements and guidelines. Specific assignments vary depending on program, grant, and patient demographics, with a focus on connecting the medically underserved to community‑based healthcare services.

Responsibilities

Develop, identify, and provide referrals within and outside of FHCSD, and effectively monitor and track such referrals.

Effectively communicate with and recruit new patients into the clinic’s or program’s system of care, interacting well with community members and patients from diverse social, cultural, and economic backgrounds.

Leverage community resources to raise awareness of FHCSD, establish and maintain good working relationships with community partners, and use these resources to connect new and existing patients with FHCSD.

Be an effective, productive, and collegial team member.

Participate in and support effective planning, development, execution, and evaluation of patient engagement plans. Investigate leads for patient recruitment and engagement opportunities.

Ensure written work (plans, flyers, reports, etc.) is accurate and timely. Use computer resources as required.

Perform other duties as assigned.

Requirements

Bachelor’s degree strongly preferred; 4 years of recent, relevant work experience may substitute.

High School diploma or GED equivalent required.

At least 1 year of experience in sales, patient engagement, community education, community health, or outreach programs is required; specific experience in program area may be required depending on assignment.

Bilingual (read, speak & write) in English/Spanish required.

Salary $25.00 - $30.93 per hour.
