Job Overview:
The role of the Intake Specialist is toassist individuals/families to explore eligibility and guide them through the process of
applying for CCO/HH services. The Intake specialist serves as the initial point of contact for agency services.
Requirements
Essential Responsibilities: Explore eligibility for services with potential individuals/families Introduce individuals/families to Tri-County Care services Direct new individuals to OPWDD Front Door sessions- the first step to access OPWDD services Assist potential individuals with the eligibility process and guide them in accessing necessary documents Work with State entities to follow up on pending approvals Navigate the HCBS waiver process with individuals/families Collaborate with the business development department to assist with outreach efforts and meeting targeted goals Field calls of interest and inquiries about Tri-County Care Refer individuals to social services agencies/community organizations available to assist them further when applicant is not eligible for Tri-County Care services, or if applicant requires assistance beyond the scope of Tri-County Care Specific Knowledge, Skills, and Abilities:
Ability to effectively communicate verbally and in a written manner Computer software skills Ability to organize, schedule, prioritize and utilize time effectively Excellent interpersonal skills, confidentiality and professionalism Required Experience
Minimum 2 years of experience working with OPWDD,
preferred Minimum 2 years of experience office experience,
required Knowledge of social services programs and community services
