Hospice Intake Specialist Job Summary The Hospice Intake Specialist plays a vital role in coordinating the referral and admission process for patients seeking hospice care. This position serves as a key point of contact for patients, families, and referral sources, ensuring a smooth, timely, and compassionate transition into hospice services. The Intake Specialist is responsible for gathering and verifying patient information, coordinating with clinical teams, and maintaining accurate documentation throughout the intake process. Key Responsibilities Referral Management:

Receive and process hospice referrals from hospitals, providers, and families, ensuring all required documentation is complete and accurate. Patient & Family Support:

Communicate clearly and compassionately with patients and families regarding hospice services, eligibility, and the admission process. Data Entry & Documentation:

Accurately enter and maintain patient information in the EMR system, ensuring all records are current and compliant. Insurance Verification:

Verify insurance coverage, benefits, and hospice eligibility; collaborate with billing teams as needed. Care Coordination:

Work closely with nurses, social workers, and external providers to facilitate timely admissions. Regulatory Compliance:

Ensure all intake processes meet Medicare, Medicaid, and state regulatory requirements. Follow-Up:

Maintain consistent communication with referral sources and families to provide updates and ensure continuity of care. Record Management:

Complete and submit all intake and admission paperwork in a timely and accurate manner. Qualifications Education:

High school diploma or equivalent required; Associate’s or Bachelor’s degree in healthcare or related field preferred. Experience:

1–2 years of experience in healthcare intake, hospice, home health, or a related setting required. Skills:

Strong organizational and multitasking abilities Excellent communication and interpersonal skills Proficiency with EMR systems and healthcare software Ability to manage sensitive information with professionalism and confidentiality Working knowledge of hospice services, insurance verification, and Medicare/Medicaid guidelines preferred

Working Conditions Office-based position, Monday–Friday, 8:00 AM to 5:00 PM May require occasional after-hours or on-call support based on organizational needs #IND4
