Documentation Specialist

The Documentation Specialist will organize, communicate, process, and document information related to hospice certifications and consents according to state and federal regulations. Additionally, he/she will process and issue all Admissions and Death Letters. Essential Functions Obtain attending/referring MD and CHPC MD signatures on hospice certifications within two (2) days of admission. This may require travel to the physicians' office and/or facility to obtain the signature. Document signatures in Suncoast. Update patient's chart with signed hospice certification. Ensuring consents are accurately signed and documented. Process and issue all Admission and Death Letters. Provides excellent customer service and maintains an amicable and professional working relationship with Community Hospice's referral sources and partners. Maintains a high level of confidentiality of all patient data as outlined in the Health Insurance Portability and Accountability Act (HIPAA). Meets the standards of the job, such as productivity, accuracy, quality standards, adherence to schedule. Ability to work overtime as needed. Performs all other duties as assigned. Qualifications Knowledge and Critical Skills Intermediate computer skills, intermediate typing and data entry. Excellent written and verbal communication skills. Excellent time management and organizational skills. Demonstrates effective people skills and sensitivities when dealing with others. Experience 5+ years customer service experience. 5+ years in a medical office environment. License / Certification Current State of Florida driver's license with driving record acceptable to Community Hospice insurance carrier. Level II ACHA (Agency for Healthcare Administration) Background Screening. Education Minimum requirement of a high school diploma; Medical degree certificate, example CNA, Medical Coding, etc...or AA degree or higher preferred.
