Patient Engagement Specialist – Full-Time Contract

Position Overview A healthcare organization is seeking a Patient Engagement Specialist (PES) to serve as a key point of contact for patient enrollment, outreach, and re-engagement in care. This role focuses on improving access to services, coordinating referrals, and strengthening patient relationships through consistent communication and follow-up.
The ideal candidate is bilingual, tech-savvy, and comfortable managing high-volume phone interactions while navigating multiple systems.
Key Responsibilities Serve as the primary point of contact for new patient enrollment and re-engagement in care Handle inbound referrals from patients and healthcare organizations via phone and web; provide information on services and schedule new patient intakes Conduct outbound calls to prospective and existing patients to promote services, schedule appointments, and address gaps in care Coordinate internal and external referrals to ensure patients are connected to appropriate services Participate in outreach initiatives and health fairs to support community engagement Deliver high-quality customer service in all interactions, including managing complex or sensitive conversations professionally Utilize phone, scheduling, and CRM systems to document call activity, manage patient data, and track engagement outcomes Follow established communication guides to effectively educate and engage patients Meet outreach and call volume expectations, including re-engaging patients who have fallen out of care Ensure timely follow-up on inbound inquiries and voicemails Maintain accurate documentation of patient interactions in approved systems Communicate effectively with internal teams and external partners while representing the organization in a professional manner
Requirements High School Diploma or GED required; college education preferred Bilingual required (Asian language strongly preferred) Minimum of 1 year of experience in customer service, call center, or sales environment required Strong written, verbal, and interpersonal communication skills Ability to manage phone conversations while navigating multiple systems simultaneously
