
Housing Specialist

The Housing Specialist (Supportive Services) is responsible for providing in‑home and community support to individuals seeking or maintaining independent housing. The role includes training in independent living skills needed to maintain a residence.
Positions identified as safety‑sensitive involve tasks or duties—including driving—that may affect the safety, health, or reputation of the Agency, participants, or others. The Housing Specialist supports an assigned caseload and documents services provided, creating a positive experience for individuals served where Kindness Matters.
Responsibilities
Provide in‑home and community support services in a trauma‑informed, recovery‑focused manner as outlined in the individual’s treatment plan.
Support patients with housing location services, including the application process and searches.
Provide education and assistance in independent living skills such as personal care, organization, food preparation, clothing care, money management, and general home cleanliness.
Provide referrals and education regarding community resources and socializing within the community.
Act as subject‑matter expert related to housing, including location services, application process, and related matters.
Coordinate care with the homeless outreach team.
Participate in LCEH meetings.
Complete clinical documentation and daily billing documentation on individuals.
Provide patients with medication education.
Provide safe, timely transportation of clients to and from various appointments as assigned.
Perform other duties as assigned.
Qualifications Required
High School diploma or GED with relevant or housing experience.
Must be 21 years of age or older.
Possess a valid Arizona driver’s license with an acceptable driving record, e.g., no serious traffic violations or more than two traffic violations in the prior 39 months.
Certification in first aid, CPR, and de‑escalation training, or a willingness to obtain such certifications.
Obtain a tuberculosis screening test upon hire and an annual screening routine thereafter.
Possess the physical, mental, and cognitive ability to perform the specific clinical and/or tasks required by the position without posing a direct threat to safety and security of patients.
Ability to type and working knowledge/familiarity with computers and electronic clinical record keeping.
Must obtain and maintain a fingerprint clearance card and pass a background check.
