* Practice Setting Office and Hospital /call for OB-GYN
* Schedule: Office hours 8a-4p for patient schedule/ call hours 24 each day

* Average number of scheduled patients per day: 25-30
* Number of Physicians in Practice: 4 Physicians (1 will be on leave) 5 midwives
* How many APP's provided coverage: 5
* Onsite Lab: Yes
* Describe Office Equipment usual OB GYN equipment, fetal dopplers, colpo, plastic and metal specs, manual BP cuffs
* Describe support staff: we have one on one clinical staff support per provider (CMA, LPN, and RN)
* What Percentage of Practice is Low Risk pregnancy: 90%
* What Percentage of Practice is High Risk pregnancy: 10%
* What Percentage of Practice is GYN: 20%
* Average number of GYN Surgeries per day (if applicable): provider preference - 1-4 depending on case time required
