- Practice Setting Office and Hospital /call for OB-GYN
- Schedule: Office hours 8a-4p for patient schedule/ call hours 24 each day

- Number of Physicians in Practice: 4 Physicians (1 will be on leave) 5 midwives
- How many APP's provided coverage: 5

- Describe Office Equipment usual OB GYN equipment, fetal dopplers, colpo, plastic and metal specs, manual BP cuffs
- Describe support staff: we have one on one clinical staff support per provider (CMA, LPN, and RN)
- What Percentage of Practice is Low Risk pregnancy: 90%
- What Percentage of Practice is High Risk pregnancy: 10%
- What Percentage of Practice is GYN: 20%
- Average number of GYN Surgeries per day (if applicable): provider preference - 1-4 depending on case time required
