- Wisconsin-registered Certified Nursing Assistant; graduate of an approved CNA training program
- High school diploma or equivalent preferred
- Prior healthcare experience preferred
- Ability to work independently and as part of a team in a fast-paced environment
- Strong communication skills, compassionate bedside manner, and professionalism
- Physically able to lift, bend, and assist residents as required
- Acceptable criminal background check
- Reports to the Charge Nurse or Shift Supervisor
- Provides routine, non-professional nursing care and support to residents in accordance with individualized care plans, facility policies, and state and federal regulations.
- Assist residents with activities of daily living, including bathing, grooming, oral care, toileting, nutrition, hydration, mobility, and prosthetic use
- Perform safe transfers, ambulation, repositioning, restorative care, and range-of-motion exercises
- Observe, document, and report changes in residents' physical, psychological, or spiritual condition
- Obtain and record vital signs, weights, and other assigned measurements
- Support admissions, discharges, and room transfers
- Encourage resident independence, dignity, choice, and participation in self-care and activities
- Maintain resident rights, privacy, safety, and infection control standards
- Provide basic housekeeping of resident areas, including bed changes
- Communicate effectively with residents, families, and the care team; participate in shift reports and care planning
- Remain flexible in assignments, assist in other areas as needed, and perform additional duties as assigned
- Maintain required training, follow schedules, dress code, and facility policies
- Ability to stand and walk for extended periods
- Ability to frequently bend, stoop, kneel, reach, and assist with transfers
- Ability to lift 35 pounds regularly and up to 50 pounds occasionally
