Responsibilities and qualifications reflect the responsibilities of this role in providing support to individuals in daily living, community integration and care planning within residential settings.
Responsibilities
Support individuals in personalized activities of daily living and social skills development in all environments (community, work, home) to help them learn, practice and master skills and engage in valued social roles.
Ensure the health, safety and well-being of individuals by following instructions from the Primary Care Physician, nursing staff, and dietary plans as applicable.
Report changes in an individual’s condition or unusual occurrences to a Supervisor or Management in a timely manner; report SIGNIFICANT changes immediately.
Report environmental risks, hazards, or barriers to Supervisor or Management in a timely manner.
Safeguard individuals’ possessions and maintain secure handling of money and personal belongings.
Assist individuals in maintaining order and cleanliness in their environments.
Provide input into planning and implementation of the individual’s plan of care, including the Support Plan and Behavior Support Plan.
Implement Individual Support Plans (ISP) and Behavior Support Plans (BSP) as written and at the frequency stated in the ISP/BSP.
Demonstrate accurate data collection procedures and use of described support strategies to promote success.
Document services provided, including individualized service goals, behavior support tracking, required notes and all other required documentation.
Support individuals to manage behavioral challenges and resolve minor problems.
Complete incident reports using the online system with all required information (who, what, where, when, etc.).
Transport individuals to medical appointments, community activities or other assigned events, ensuring safe transport and adherence to traffic laws.
Maintain timely and effective communication regarding individuals’ needs and environment with appropriate personnel.
Serve as a role model and mentor, demonstrating socially appropriate behaviors and coaching individuals using approved techniques; use approved intervention techniques when necessary to protect individuals and others.
Support each individual’s existing relationships and help expand their networks of friends and acquaintances.
Encourage individuals to explore and express interest in community membership.
Observe and maintain confidentiality in accordance with HIPAA and agency requirements.
Complete scheduled training by the announced deadline and participate in home staff meetings.
Participate in the agency’s ongoing efforts to improve the quality of services and the effectiveness of operations by sharing ideas and insights.
Foster teamwork through open communication, shared responsibilities and a positive, constructive attitude.
Interact professionally with colleagues, families, guardians, support coordinators and internal/external monitors.
Complete other duties as assigned.
In Residential Settings Only
Provide assistance with activities of daily living, including cooking, cleaning, laundry, household maintenance, community involvement, personal hygiene, health maintenance, medication administration/medication management and social/behavioral development.
Document services provided (ISP goals, behavior support tracking, notes, communication logs, MAR and other required documentation).
Maintain proper communication regarding individuals’ needs and home environment with appropriate personnel; read and update the communication log on shift changes.
Plan, prepare and grocery shop for meals, following special diets and nutritional instructions.
Follow Supervised Self Administration of Medication (SSAM) and Proxy Caregiver procedures; read and follow medical and nursing instructions.
Required
High school diploma or GED and any combination of training and experience to acquire necessary knowledge, skills and abilities.
Possess Georgia Driver’s License and acceptable Motor Vehicle Record (MVR).
Basic computer skills, ability to advocate for individuals, interpret written materials, prepare incident reports and strong conflict resolution skills.
Physical Requirements
The physical demands described here are representative of those required to perform essential functions.
Regular lifting and carrying up to 30 pounds unassisted, including assisting individuals when required.
Frequent bending, reaching, repetitive hand movements, standing, walking, squatting and sitting, with some heavy lifting, pushing and pulling during a shift.
Preferred
Six (6) months experience working with the intellectually and developmentally disabled population.
Salary Range
$16.36 Hourly
