Job Summary
The role leads daily operations for community-based programs, collaborating closely with internal leaders and external community partners to address the social factors that influence health outcomes. This position supports the design, implementation, and management of programs that respond to identified community needs. It also plays a key role in driving innovation in care delivery, introducing new operational approaches, and guiding the team through organizational changes to improve service quality and community impact.

Qualifications
Education
BA in Public Health, Health Administration, Social Work, Community Health, Healthcare Management, or similar scope of education
Experience
5 years of experience in community health, population health, or public health program management
3 years of supervisory or leadership experience within a healthcare or community-based setting.
Participated and partnered in program development, implementation, and evaluation with a focus on health equity and community engagement.
2 years experience in grant management, including proposal writing, reporting, and funder relationship development.
2 years experience working with diverse stakeholders, including healthcare providers, community-based organizations, and public agencies.
2 years experience and understanding of the Social Drivers of Health (SDOH) and their impact on health outcomes.
2 years experience with budget development and financial management within a program or department.
Language Skills
Bilingual in Spanish/English
Knowledge, Skills And Ability Requirements
Excellent leadership, communication, and interpersonal skills, with the ability to motivate, develop teams and amplify community voices.
Excellent collaboration and relationship skills
Strong organizational, project management, and problem‑solving abilities.
Proficiency in data collection, reporting, and outcome measurement to support performance improvement and community benefit reporting.
Demonstrated ability to work effectively in a complex environment with community stakeholders and across multidisciplinary teams.
