Since 1969, Lakeside has been the premier provider of specialized behavioral health and addictive disease services in the Memphis and Mid-South region. The 365-bed facility is one of the largest free-standing psychiatric hospitalsin the United States.We arelocatedon a serene 37-acre campus in northeast Shelby County, serving as the setting for the treatment for children, adolescents, young adults,adultsand seniors. Lakeside's comprehensive programs treat disorders such as bipolar disorder, psychological trauma, depression and anxiety, mental illness and addictive disease disorders such as alcoholism, drugaddictionand substance abuse.
JOB SUMMARY:
The Discharge Planner provides discharge planning and case management services, crisis intervention and liaison services with referral sources and other departments within the hospital in a manner consistent with the philosophy and objectives of Lakeside Behavioral Health System.

JOB SPECIFICATIONS:
To perform this job successfully, an individual must be able to demonstrate competency in the criteria listed in this job description. The specifications listed below are representative of the knowledge, skill, and ability to meet the minimum requirements for this position. Staff must demonstrate flexibility for reassignment to units providing care to varied populations including, adults and dually diagnosed patients in an inpatient and partial hospital setting.
Education: Bachelor's degree in Social Work, Clinical/Counseling Psychology or Social Science related area required, licensure preferred.
Graduate of an accredited nursing program.
Experience: Prior experience working with a psychiatric mental health population, preferably in an inpatient setting.
Licensure/Certification: If a Registered Nurse - must have licensure and registration by the Tennessee Board of Nurse Examiners as a
registered nurse.
Knowledge: Prefer knowledge of case management, discharge planning, family therapy theory and practice.
Supervision Received: Director of Social Services
Supervision Exercised: N/A
Environmental Conditions:
Exposure to psychiatric patients who may exhibit violent/aggressive behavior. Potential for exposure to communicable diseases, blood/body fluids and other hazardous waste. General hospital environment.
