Full-Time | 9:00 AM – 5:00 PM
Salary:
$60,000 annually
Responsibilities
Conduct psychosocial assessments for new admissions, readmissions, and during quarterly, annual, and significant status changes
Develop and implement individualized care plans based on residents’ needs, preferences, and goals
Support residents and families in adjusting to nursing home life with empathy and professionalism
Advocate for residents’ rights and ensure their voices are heard in care decisions
Facilitate communication between residents, families, and the interdisciplinary care team
Coordinate discharge planning, including community referrals, home health services, and follow-up care
Provide counseling and crisis intervention as needed
Assist with advanced directives, guardianship, and end-of-life planning
Participate in care plan meetings, resident council, and UR meetings as needed
Maintain accurate documentation in compliance with state, federal, and facility standards
Educate staff on psychosocial issues, resident rights, and effective communication strategies
Complete audits as assigned
Manage distribution of wander guard devices and track advanced directive updates
Ensure residents are invited to care plan meetings in advance
Qualifications
Education: Bachelor’s or Master’s Degree in Social Work (BSW or MSW) or a related field from an accredited institution
Experience: Minimum 1–2 years of experience in long-term care, geriatric, or healthcare social work required
Experience with SigmaCare and proficiency in Microsoft Word preferred
Strong communication, organizational, and interpersonal skills
