Responsibilities
Conduct formal OT evaluations and develop individualized treatment plans for residents
Implement evidence-based therapy to improve activities of daily living, mobility, upper-extremity function, and cognitive engagement as appropriate
Reassess residents regularly; adjust treatment plans and document progress notes in the chart
Collaborate with nursing, physical therapy, speech, social work, and other team members to coordinate resident care
Educate residents and families on home programs, safety, and adaptive strategies
Ensure documentation accuracy and compliance with facility policies and regulatory requirements
Requirements
Active California Occupational Therapist license in good standing
Master’s or Doctorate degree in Occupational Therapy from an accredited program
Experience in a skilled nursing facility or geriatric rehabilitation setting preferred
Strong assessment, treatment planning, and documentation skills
Ability to work effectively in a fast-paced, multidisciplinary SNF environment
Willingness to commit to a 13-week travel assignment starting May 18, 2026
