Responsibilities
Perform initial PT evaluations and reassessments to determine patient needs and goals
Develop, implement, and adapt individualized treatment plans to support functional recovery and independence
Administer therapeutic interventions including therapeutic exercises, manual therapy, and modality applications as indicated
Document patient progress, treatment plans, and outcomes in alignment with facility and regulatory guidelines
Collaborate with nursing, physicians, and other therapy staff to coordinate care and optimize resident outcomes
Ensure safety, infection control, and equipment handling in all therapy sessions and clinical activities
Requirements
Active or eligible Physical Therapist license to practice in Pennsylvania
Doctor of Physical Therapy (DPT) degree from an accredited program (or equivalent)
Ability to work in a long‑term care setting and participate in a travel assignment
Strong communication and documentation skills; ability to collaborate with an interdisciplinary team
Availability to work the scheduled shift (08:00–16:30) and adapt to the assignment’s travel requirements
