Conduct initial physical therapy evaluations and ongoing reassessments for residents in a long‑term care setting
Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence
Deliver evidence‑based PT interventions within scope and facility policies; monitor and document progress; adjust Plan of Care as needed
Document evaluations, goals, progress notes, and discharge plans in patient records, ensuring accuracy and regulatory compliance
Collaborate with an interdisciplinary team (nurses, therapists, physicians) to coordinate care and support transitions of care
Educate residents and families on home exercise programs, safety strategies, fall prevention, and activity modification
Requirements
Active physical therapy license to practice in Maryland (or eligibility to obtain licensure)
Ability to work a 38‑hour week on a 08:00–16:30 schedule in a long‑term care setting
Willingness to travel for temporary assignment and support a 13‑week contract
Strong communication, documentation, and teamwork skills to coordinate with the care team
Demonstrated ability to work in a regulated healthcare environment and adhere to facility policies
