Overview
Physical Therapist – Long-Term Care / Nursing Home setting in Siler City, North Carolina. This travel assignment starts on 04/27/2026 and runs for 13 weeks with guaranteed 35 hours per week. Responsibilities
Assess residents’ physical therapy needs and develop individualized treatment plans
Implement, monitor, and adjust therapy interventions to improve mobility, strength, balance, and functional independence
Reassess progress regularly; document patient status, treatment goals, and outcomes in charting
Collaborate with an interdisciplinary team to coordinate care plans and discharge planning
Educate residents and family members on exercises, safety strategies, and home programs; ensure patient and family understanding of plan of care
Maintain safe, compliant clinical practice and adhere to all relevant policies and infection control guidelines
Requirements
Active Physical Therapist license in North Carolina or eligible for licensure
Current CPR/BLS certification
Ability to practice in a long‑term care setting and work with older adults
Strong communication, documentation, and teamwork skills
Availability to work 35 guaranteed hours per week on a 5x8 day schedule (08:00–16:30)
