Responsibilities

Conduct OT evaluations and develop individualized treatment plans for residents

Implement and monitor therapy programs to improve activities of daily living, mobility, cognition, and safety

Reassess resident progress and adjust treatment plans as needed; document outcomes in records

Collaborate with an interdisciplinary care team (nursing, therapy disciplines, social work) to coordinate care

Educate residents and families on home exercise programs, equipment, and safety strategies

Ensure compliance with regulatory requirements, payer guidelines, and facility policies; maintain infection control and safety standards

Requirements

Active occupational therapy license in Maryland

Eligible to work in the United States

Degree in Occupational Therapy from an accredited program

Ability to work in a skilled nursing facility environment

Experience in geriatric rehabilitation or SNF settings preferred

Demonstrated professionalism, communication, and documentation skills; ability to work collaboratively with a multidisciplinary team
