Position Overview
The Community Hospice offers a Per Diem LPN position on the Hospice Home Care Team, caring for patients and families in both homecare and nursing home settings. Primary region: Saratoga and Washington Counties. Weekdays 8 am‑4 pm. No weekends or on‑call duties.

What You Will Do
Coordinate closely with our RN Supervisor and RNs to provide nursing and personal care services as part of a comprehensive plan of care to support patients and families.
Work closely with a multidisciplinary team including RNs, aides, medical directors, social workers, chaplains, complementary therapists, and volunteers.
Receive training in end‑of‑life care.
What You Will Need
Valid NYS LPN license.
Minimum 1 year LPN experience.
Prior hospice/home‑care experience preferred (not required).
Strong computer skills.
Organization and communication skills; ability to work as part of a team.
Benefits and Compensation
Enhanced rates for Per Diem status.
Pay Range for non‑per‑diem status: $25.00-$31.10 per hour. Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may vary within the stated range based on location.
