We are seeking an experienced **Registered Nurse (RN) Case Manager** based in **Clanton, Alabama** to coordinate and manage the continuum of care for assigned patients across surrounding counties. This role focuses on high-quality, cost-effective care delivery while advocating for patient needs and ensuring compliance with individualized care plans.
**Position Details**
+ **Pay Rate:** $52.05/hour
+ **Contract Length:** 9 weeks
+ **Hours:** 40 hours/week
+ **Schedule:**
+ Monday-Friday, 8:00 AM-5:00 PM
+ Flexible day off (Monday _or_ Friday)
+ **On-Call:** Every other weekend (very minimal calls/visits)
+ **Shift Days:** Weekdays & rotating weekends
+ **Mileage Reimbursement:** $0.48 per mile
+ **Coverage Area:** Wilcox, Dallas, Marengo, Clarke counties
**Primary Responsibilities**
+ Perform comprehensive assessments of high-risk patients
+ Collaborate with primary care providers to implement individualized care plans
+ Reassess and manage appropriate, cost-effective levels of care
+ Coordinate transitions of care across settings
+ Arrange non-medical services (housing, transportation, etc.) to support treatment compliance
+ Partner with physicians, social workers, discharge planners, and claims professionals
+ Engage specialty resources to ensure optimal outcomes
+ Maintain accurate documentation of clinical, functional, and fiscal outcomes
+ Promote health education, illness prevention, and early intervention
+ Advocate for patients while ensuring privacy and confidentiality
**Minimum Requirements**
+ Current **Alabama RN license** (unrestricted)
+ **1+ year Case Management experience** (preferred)
+ Associate or Bachelor's Degree in Nursing or related field
+ Case Management Certification
+ Current CPR (if required)
+ TB screening (questionnaire, PPD, or chest X-ray as applicable)
+ Current health certificate per state or contract requirements
+ Must meet all federal, state, and local requirements
+ Must be at least 18 years of age
**Preferred Skills**
+ Home Health experience highly preferred
+ Experience with:
+ IV therapy
+ Wound Vacs
+ OASIS documentation
+ Ostomies
+ Foley catheters
+ Chest tubes
