Participate in interdisciplinary team meetings to ensure coordinated, high‑quality care
Provide compassionate, culturally sensitive care with a strong customer‑service mindset
Perform additional duties within LPN scope of practice as assigned
Qualifications
Active Massachusetts LPN license in good standing (required)
2+ years of nursing experience preferred (new grads encouraged to apply)

Ability to work independently and manage time effectively in community‑based settings
Sound clinical judgment and strong critical‑thinking skills
Intermediate proficiency with Microsoft Office Suite
Experience with EMRs and clinical documentation systems
Ability to use telehealth tools (video, chat, etc.) as needed
Prior case management experience preferred but not required
