*What You’ll Do*
* Perform in-home evaluations and develop individualized treatment plans.
* Deliver evidence-based occupational therapy to help patients regain independence and improve daily living skills.
* Educate patients and caregivers on adaptive strategies, assistive devices, and home modifications.
* Collaborate with nurses, PTs, and other team members to ensure coordinated care.
* Maintain accurate, timely documentation in compliance with home health standards.

* Licensed Occupational Therapist (OTR/L) in the state of Michigan.
* Home health experience preferred, but not required.
* Strong clinical skills and a commitment to patient-centered care.
* Reliable transportation and willingness to travel to patients’ homes throughout our service area (*approximately a 50-mile radius from Gaylord*).
