What You’ll Do
Assist the Social Services Director in implementing and carrying out social service programs in accordance with state and federal guidelines
Support the identification and documentation of residents’ social, emotional, and psychological needs
Provide service coordination and support for residents and families, including communication and follow-up
Participate in care planning and interdisciplinary team meetings as assigned
Maintain accurate, timely documentation to support compliance and regulatory standards
What You Bring
Associate’s Degree in a Human Services, Social Work, Psychology, or related field preferred, but not required (experience may be considered in lieu of degree)
Experience working in a skilled nursing or long-term care setting preferred
Strong written and verbal communication skills, with the ability to interact professionally with residents, families, and interdisciplinary teams
Ability to assist with documentation, assessments, care planning, and regulatory requirements under the direction of a licensed Social Services Director
Compassionate, resident-focused approach with strong interpersonal skills
Organized and detail-oriented, with the ability to manage multiple priorities
Working knowledge of resident rights, discharge planning, and psychosocial support services preferred
Positive, can-do attitude with excellent attendance
