Social Services Assistant – Skilled Nursing & Long-Term Care Experience Preferred

Position Summary : The
Social Services Assistant
will support the Director of Social Services in providing essential services to residents and families. This role ensures that residents’ social, emotional, and psychosocial needs are met in compliance with state, federal, and facility guidelines.
Key Responsibilities
Assist with resident assessments, care plans, and documentation
Provide support during admission, transfer, and discharge processes
Help residents and families adjust to long-term care placement
Advocate for residents’ rights and quality of life
Facilitate communication between residents, families, and the interdisciplinary care team
Assist in organizing and implementing programs that support residents’ social and emotional well-being
Maintain accurate records and documentation
Qualifications
High school diploma or equivalent (Associate’s or Bachelor’s in Social Work, Psychology, or Human Services preferred)
Experience in long-term care, healthcare, or social services field preferred
Strong communication, organizational, and interpersonal skills
Compassionate, patient, and resident-focused approachAbility to maintain confidentiality and professionalism
