Job Summary
Unity Health and Rehabilitation Center, a leading provider of rehabilitation and skilled nursing care, is seeking a compassionate and dedicated Social Services Assistant to join our team in Miami, Florida. Responsibilities
Provide emotional support and social services to patients, residents, and their families
Conduct social histories and assessments to identify patients' social and emotional needs
Develop and implement individualized care plans to meet patients' social and emotional needs
Facilitate group activities and events to promote socialization and a sense of community
Collaborate with interdisciplinary teams to ensure comprehensive care and support
Maintain accurate and up-to-date records and reports
Participate in ongoing education and training to stay current on best practices in social work and rehabilitation
Requirements
Education: High School Diploma, Associate degree or higher in social work, human services, psychology, gerontology, sociology, counseling, or a related field preferred. Completion of training on long‑term care social service practice, resident rights, psychosocial assessment, and care planning is strongly preferred.
Experience: Must have, as a minimum, one (1) year of experience in a healthcare or long‑term care setting preferred.
