Job Description
Director of Social Work
- Hopkins Center for Rehabilitation (288-bed skilled nursing facility in Brooklyn, NY) seeks a qualified and dedicated professional to join our dynamic team. The facility offers a full continuum of care, from temporary respite stays, to short‑term rehabilitation, to long‑term skilled nursing care, and a broad array of specialty programs and services.
Job Duties / Responsibilities
Knowledge of age‑specific developmental factors for adult and geriatric residents (physical, cognitive, socialization) and directing the overall operation of the facility's service delivery.
Perform resident evaluations required by state regulations.
Support state regulations for admission and retention of residents.
Assist residents and families with the move‑in process and orient them to the facility.
Assist and interact with residents and families to access services.
Discharge planning and assistance with the move‑out process.
Participate in development and implementation of each resident’s individual service plan.
Communicate with other members of the management team to ensure coordination of services.
Document case management notes in residents’ files as necessary.
Perform other duties as assigned (when required).
Knowledge of MDS and discharge planning is essential.
Qualifications
MSW.
Experience in the field of social work providing services to an adult population as required by state regulation.
Preferred: 2–5 years of experience in adult populations.
Computer knowledge, particularly in Microsoft Word and Excel.
Certified/licensed as required by state regulations.
Bilingual preferred.
Work Authorization
United States (Required).
Job Type: Full‑time
Pay: $100,000.00 – $120,000.00 per year
