Key Responsibilities
Utilize the nutrition care process to conduct comprehensive assessments for residents upon admission and routine reassessments.
Facilitate nutrition related care for residents in short‑term rehab and long‑term care units.
Collaborate with the interdisciplinary care team to determine appropriate plans of care for residents.
Monitor residents’ dietary intake, weight, lab values and habits.
Provide nutrition education and counseling to residents and families.
Participate in care plan meetings, quality improvement initiatives, and nutrition programs.
Ensure compliance with state, federal, and facility regulations.
Complete MDS assessment in compliance with state guidelines.
Collaborate with dietary staff for menu planning and oversee food‑service operations as needed.
Qualifications
Registered Dietitian (RD) credential required.
Bachelor’s degree in Dietetics, Nutrition, or a related field.
Current state licensure (or eligibility) in New York.
Preferred: previous experience in a clinical setting.
