The Medical Social Worker reports to the Clinical Manager and participates in interdisciplinary care for home health patients. The role involves evaluating and developing personalized plans of care to address patients’ emotional and social needs, supervising the Social Worker Assistant when involved in the patient’s plan, and adhering to CenterWell Home Health policies, applicable healthcare standards, and government regulations.
Responsibilities
Assess the patient’s social and emotional state as it relates to their illness or injury, their needs for care, and their response to treatment, and adjust care accordingly.
Assess relationships among the patient’s medical and nursing needs in the home setting, financial resources, and available community resources.
Take appropriate action to obtain community resources to help resolve issues that may impede the patient’s recovery.
Communicate with patients, their family members, and associates in the organization.
Provide ongoing assessment of patient and family needs and responses to understanding the treatment process and coping with social and emotional reactions.
Participate as a member of the interdisciplinary care team in care‑coordination activities and act as a resource to other health team members in identifying and resolving patient needs.
Evaluate the performance of the Social Work Assistant (BSW) to assure that all medical social services are provided to patients in compliance with company, government, and professional standards.
Maintain and submit required documentation, including case conferences, patient/physician community contacts, visit reports, progress notes, and collaboration with other health‑care disciplines to provide optimum patient care.
Required Qualifications
Minimum of one year of experience as a social worker in a health‑care setting, home health, and/or hospice.
Social Worker licensure in the state of practice, if required by state law or regulation.
A valid driver’s license, auto insurance, and reliable transportation.
Proof of current CPR certification.
Experience with discharge planning and obtaining community resources (housing, shelter, funeral/memorial services, legal, information and referral, state/federal financial and medication programs, and eligibility).
Knowledge of medications and their correct administration.
Demonstrated ability to organize tasks, develop action plans, set priorities, and function under stressful situations.
Flexibility in work hours and local travel.
Maintain current licensure certifications and meet mandatory continuing education requirements.
Must read, write, and speak fluent English.

– This role is part of Humana’s driver safety program and requires a valid state driver’s license and personal vehicle liability insurance. The individual must carry vehicle insurance in accordance with state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 property damage, or whichever is higher.

Pay Range
– $65,000–$88,600 per year (full‑time, 40 hours/week). Pay may vary based on geographic location and individual qualifications.
