Medical Social Worker
Responsibilities

Assesses the patient’s social and emotional state as it relates to illness/injury and adjusts care accordingly.

Evaluates relationships of the patient’s medical and nursing needs in the home setting, financial resources, and available community resources.

Obtains community resources to resolve issues impeding patient recovery.

Instructs patients and families in coping with social and emotional responses and provides ongoing assessment of patient and family needs and responses to teaching.

Assists physicians and other health team members in understanding significant social and emotional factors related to patient health; participates in development and periodic re-evaluation of the physician’s Plan of Care.

Observes, records, and reports changes in patient condition and response to treatment to Clinical Manager; participates in discharge planning.

Participates as member of interdisciplinary care team in care coordination activities; acts as resource to other health team members in identifying and resolving patient needs.

Supervises, instructs, and evaluates performance of Social Work Assistant (BSW) to ensure compliance with company, government, and professional standards.

Maintains and submits required documentation, including case conferences, patient/physician community contacts, visit reports, and progress notes.

Qualifications

Master’s or doctoral degree from an accredited school of social work (Council on Social Work Education).

Social Work license in the state of practice, if required by state law.

Valid driver’s license, auto insurance, and reliable transportation.

Current CPR certification.

Minimum one year of experience as a social worker in a health care setting (home health, hospice, or similar).

Knowledge of discharge planning and ability to obtain community resources (housing, shelter, funeral/memorial services, legal, information and referral, state/federal financial and medication programs).

Excellent oral and written communication and interpersonal skills.

Fluent English reading, writing, and speaking.

Knowledge of medications and their correct administration.

Organizational and prioritization skills; ability to function under stressful situations.

Flexibility in work hours and local travel.

Strong communication with patients, families, and organization at all levels.

Maintains current licensure, certifications, and meets mandatory continuing education requirements.

Good and regular attendance.

Ability to perform other related duties as assigned.

Scheduled Weekly Hours
40 hours per week.

Pay Range
$65,000 - $88,600 per year.
