
Medical Social Worker Hospice

Responsibilities
Establish a care plan in accordance with the physician’s treatment plan.
Provide crisis intervention as required.
Assist the physician and other caregivers in understanding the significant social, emotional, and economic factors influencing the patient’s ability to improve medically.
Provide patient care education to patients and caregivers.
Assist in making appropriate community referrals to gain access to additional resources as needed (e.g., applications for public assistance, housing problems, additional healthcare resources, transportation).
Work with the patient’s caregivers to resolve problems interfering with an optimum caring relationship between the patient and caregiver.
Complete and submit all required documentation accurately and in a timely manner.

Qualifications
Master’s degree from an accredited school approved by the Council on Social Work Education. In Indiana, must also be certified under IC25-23.6-4-3.
Currently licensed to practice in the state served.
One year of social work experience in a health care setting.
Current CPR/BLS certification through American Heart Association or American Red Cross and compliance with state requirements.
