Job Description
We’re looking for Social Services Assistants who are passionate about supporting residents and their families through meaningful care and guidance. In this role, you’ll help coordinate and implement social service programs, assist with discharge planning, and provide support to residents as they navigate personal, emotional, and environmental challenges. You’ll play an important role in creating a supportive and respectful environment while collaborating closely with care teams.
Key Responsibilities
Conduct admission interviews and complete psychosocial assessments and histories.
Develop and maintain individualized social service care plans and goals.
Assist residents and families with transitions, including admission and discharge processes.
Ensure Advanced Directives and POLST documentation are complete and properly maintained.
Coordinate ancillary services such as dental, optical, audiology, and podiatry.
Participate in resident care conferences and interdisciplinary team (IDT) meetings.
Maintain ongoing communication with residents and families to address needs and concerns.
Document progress notes and complete required documentation, including MDS sections.
Support discharge planning, including home care needs and transportation arrangements.
Advocate for resident rights and serve as a liaison between staff, families, and community agencies.
Ensure compliance with Title 22, OBRA, and other state and federal regulations.
Process Medicare, HMO, and private denial letters in accordance with regulations.
Collaborative Teamwork
This role involves working closely with interdisciplinary teams, residents, and families to ensure coordinated, compassionate care. You’ll contribute to a supportive and collaborative environment focused on improving quality of life for each resident.
Qualifications
High School Diploma or GED required.
Previous experience in social services or skilled nursing preferred.
Strong organizational skills with the ability to multitask.
Excellent verbal and written communication skills.
Ability to read, write, speak, and understand English.
Ability to make independent decisions when appropriate.
Must demonstrate Service Excellence in interactions with patients, families, coworkers, and the community.
