Team Coordinator
to join our dynamic home health care team. The
Team Coordinator
supports the clinical operations and management staff members.
Essential Job Functions
Greet and assist clients, employees, and general public when entering office providing excellent client and employee customer service
Provide daily analytical support and validation through our EMR system (Homecare Homebase) per industry regulations
Daily activities can include but are not limited to:
Scheduling clinicians, notify physicians of care changes, perform pre-bill auditing
Send patient instructions
Verify receipt of proper documentation and follow up as necessary with clinicians, clients and families
Manage office calls, correspondance and meetings
Monitor office supply levels
Provide customer support
Support ongoing client and employee requests including staff scheduling, preparing client materials, administrative tasks and processing of client files in alignment of government/health care standards
Work collaboratively with all branches and home office to ensure seamless coordination and service amongst our business and clinical delivery team
Participate in staff meetings and meets regularly with branch management for individual supervision to meet performance objectives
Comply with HIPAA and Data Privacy Practices and regulation

Requirements
Proficient with Microsoft Suite
Customer service experience
Excellent written and verbal communication skills
Preferred
Prior scheduling Home Health scheduling experience
Prior scheduling LTC experience
Demonstrated proficiency in clinical assessments, documentation and compliance with policies and procedures
